According to state health law, students entering the Oswayo School District and students in 6th and 11th grade should receive a physical. While the school district will provide a free physical, your private physician is most familiar with your child’s health and family history. Immunizations can also be addressed during the physical. For these reasons it is advisable that the mandated physical to be done by your private practitioner.
Click on the link below for the Private Physical Form. Any physical exam that your son or daughter had or will have from September 2019 to May 2021 will be in the acceptable time frame. If your child has been seen or will be seen by a physician, physician’s assistant or nurse practitioner during this time period, please ask them to complete the form below.
The form can then be brought to the main office or be faxed to 814- 697-7799 (Elementary School) or 814-697-6375 (Middle/High School) If your physician’s office says they will fax the form, please verify that the Oswayo Valley Health Office has received the form.
Your child will be scheduled for a school physical unless you have either notified the health office of a scheduled physical date or a completed Private Physical Form is returned by February 15, 2021.
