According to state health law, every child upon entering school (Pre-K or K and new students to the district) should receive a dental exam. Likewise, children in third grade and seventh grade are required to be seen by a dentist. Any dental appointment that your child has had from September 2019 to May 2021 will fulfill this requirement.
Please see the link below for a copy of the Private Dental Form. If your child has been or will be seen by your private dentist, please return the completed form to the health office by January 15, 2021. Forms may be faxed to 814-697-7799 (Elementary School) or 814-697-6375 (Middle/High School). If you have an appointment scheduled before May 1, 2021 kindly notify the health office of that date.
In the event that you do not have a private dentist, there are two opportunities at the school for your child to be seen by a dentist.
The Mobile Dentist will visit the Oswayo Valley School District in the spring of the 2020 – 2021 school year. This dental exam may include cleaning, fluoride, X-rays, and sealants. Restorative care (fillings) and extraction of primary teeth may also be done.
Dr. Blake, the school dentist, will visit in February 2021. He will identify any cavities and need for orthodontics.
Your child will be scheduled to receive a free school dental exam in February 2021 unless:
· A private dental form is completed and returned by January 15, 2021
· Your child will be seen by the Mobile Dentist
· You have notified the health office by January 15, 2021 of an upcoming dental appointment.
